Privacy Statement

Not only is your treatment pathway handled with the greatest possible care at RealHealth, but so too are your personal data. After all, you don't want your data to end up in the wrong hands. Below you will find an overview of the personal data we process:

Passport photograph

To quickly have a picture to associate with the file when you call or when consultations take place.

First and surname

If you become a patient, we must know who you are. After all, we want to be certain that the right person receives the right treatment.

Date of birth

Your date of birth is necessary to establish your identity and we need your date of birth to be able to communicate internally and with other healthcare providers.

Address details

We need your address details to be able to send you information by post if necessary.

Telephone number

We need your telephone number because we call you to schedule appointments. Additionally, an appointment may be cancelled due to unforeseen circumstances. We want to be able to reach you quickly.

Email address

We need your email address because you receive confirmations and reminders for appointments here. We also use your email address to send questionnaires relating to scientific research.

Specific and general questionnaires

These are sent in advance and are used for scientific research and to evaluate our programme. This questionnaire is administered at four measurement points (before the intake, on the last training day, after one month and after one year) and provides insight into the progression of complaints over time. It shows which aspects our programme has had an effect on. In addition, we use this information partly to prepare for the intake to assess whether our programme is suitable or not. The choice of questionnaires was made based on the ICHOM Low Back Pain (International Consortium for Health Outcomes Measurement), so that the results can also be compared internationally. When processing data relating to scientific research, data is processed anonymously.

RealHealth processes the following special and/or sensitive personal data from you:

Health

Your treatment can only proceed safely if we know all information about your health. Think, for example, of heart conditions and allergies.

Citizen Service Number (BSN)

Healthcare providers such as RealHealth are obliged to record the citizen service number (BSN) of their patients in their administration. This is stated in the Act on the Use of Citizen Service Numbers in Healthcare (Wbsn-z). The BSN in healthcare is used to:

  • prevent errors in the exchange of financial and medical data;

  • simplify claims with health insurance;

  • prevent mistaken identity;

  • offer better protection against identity fraud.

Healthcare providers must verify that you are the person associated with the Citizen Service Number (BSN). That is why recognised healthcare providers ask patients for valid proof of identity.

For what purpose and on what basis we process personal data

RealHealth processes your personal data for the following purposes:

  • To be able to treat you as safely and effectively as possible. If RealHealth is not properly informed about someone's health, this can have consequences for treatment.

  • To be able to call or email you if this is necessary to be able to carry out our services.

  • To properly organise your stay during treatment, RealHealth provides your name and address details to the hotel with which we collaborate.

  • RealHealth also processes personal data if we are legally obliged to do so, such as data that we need for safe medical treatment.

How long we retain personal data

RealHealth retains your personal data in accordance with the statutory retention period 'or as much longer as follows from the duty of care of a good healthcare provider'. A healthcare provider is only able to provide (continuing) good care if the data is retained for a long time.

Sharing personal data with third parties

RealHealth does not sell your data to third parties and only provides it if this is necessary for the performance of our agreement with you or to comply with a legal obligation. With companies that process your data on our instructions, we enter into a processing agreement to ensure the same level of security and confidentiality of your data. RealHealth remains responsible for these processing activities.

Your visit to realhealth.nl is a safe visit

A cookie is a small piece of information that is stored on the computer in the form of a file. Cookies can be divided into tracking cookies and technical cookies. RealHealth does not use tracking and/or technical cookies.

Viewing, amending or deleting data

You have the right to view, correct or delete your personal data. In addition, you have the right to withdraw your consent for data processing or to object to the processing of your personal data by RealHealth, and you have the right to data portability. This means that you can submit a request to us to send the personal data we hold about you in a computer file to you or to another organisation you specify. When you ask us to forward your data to another organisation, we only forward the data relating to the relevant indication. You can send a request for inspection, correction, deletion, data transfer of your personal data or a request to withdraw your consent or object to the processing of your personal data to realhealth@maartenskliniek.nl. To ensure that the request for file inspection has been made by you, we make an appointment with you at our location and ask you to bring a copy of your proof of identity. We will respond to your request as quickly as possible, but within four weeks. RealHealth also wishes to point out that you have the possibility to lodge a complaint with the national supervisory authority, the Dutch Data Protection Authority. How we secure personal data

RealHealth takes the protection of your data seriously and takes appropriate measures to prevent misuse, loss, unauthorised access, unwanted disclosure and unauthorised modification. Data Protection Officer

If there is a suspicion that RealHealth is handling your personal data carelessly, unsafely or not in accordance with the law, you can contact the Data Protection Officer (DPO) of Maartenskliniek. They can be reached via privacy@maartenskliniek.nl. Furthermore, you can pass on your questions about personal data, requests for your own file, inspection and amendments of personal data to the secretariat of RealHealth, tel. 024-6841640.
